Step 1
Contact Us
Call us during business hours to discuss your condition. We’d be glad to answer your questions and schedule an in person visit. You can alternatively email us your questions. Walk-ins are welcome. Please be sure to respect and abide by all COVID-19 precaution measures. We can not admit patients with COVID-19 symptoms. Step 2
Registration Forms, Insurance & Financing
After making your appointment, please print and complete below forms as best you can. We need this information to register you in our patient management system. It will be a good idea to email them to info@bolouridental.com for processing prior to arrival.
Registration & Medical History Form
We accept the following insurance plans: Aetna, Ameritas, BlueCross BlueShield – BCBS, Delta Dental, Guardian, GEHA, Humana, Metlife, MODA, Principal Financial, Regence, State Insurance Oregon Health Plan ODS/OHP, Family Dental Care, and MBA. We also accept members of the Teamsters Union. Please call us to verify if we are registered with your plan.
Financing services are available if you’d like to take advantage of deferred monthly payments. We work with CareCredit for dental financing. For the most part, you should be able to finance most preventive dentistry, restorative procedures and treatments.
Step 3
First Visit
Please plan to arrive 15 minutes earlier. Do not forget your registration forms if you haven’t emailed them. Please be sure to ware a facemask and take all necessary COVID-19 related precautions. Please contact us and avoid visiting the clinic if you are experiencing any COVID-19 symptoms. We’ll gladly reschedule your appointment.
During your fist visit, we will review your dental and medical history, perform a full examination and evaluate your dental condition. In most cases, a hygienist will clean and polish your teeth, take X-rays, and inspect your teeth for signs of cavities and examine the health of your gums and bone around each tooth . Finally, one of our dentists will review your X-rays and examine your teeth along with hygienist report to inform you of your dental health. With your input, we will craft a prioritized treatment plan and schedule subsequent visits to correct and treat your specific conditions. This plan will include the work to be performed, costs, insurance coverage and copays.
Step 4
Regular Checkups
It is necessary to monitor your dental health on semi annual basis to verify that all is well. Our goal is to identify and treat issues early before severe damage to your teeth or gums.
Step 5
Emergencies
For dental related emergencies, call us during business hours to schedule immediate care. You will receive recorded instructions on what to do when the clinic is closed.
